ABCM Board Certification Exam Application

Follow 3 Easy Steps:

1) Download the exam application, fill it out and submit electronically with the submit button, or submit by mail along with your check or money order to ABCM, P.O. Box 3395, Riverview, FL  33568, or pay online on the Exam Application page.

2)  A representative from ABCM will then contact you with the name and location of your exam proctor.

3)  You make contact with the ABCM proctor to set up a date and time suitable to both of your schedules.
